Anonymous Reporting of Compliance Concerns

In addition to the Comment Box located near the employee entrance, the Hospital shall provide this compliance reporting form, for employees to submit suggestions and/or constructive comments about items of concern. This form shall also be used to anonymously report non-compliance issues to the Compliance Officer. Each person is responsible for acting in an ethical manner. Employees are expected to report issues timely (within 5 days) so we can take actions to help reduce any potential harm to patients & liability to the hospital. When reporting issues anonymously, please provide sufficient details regarding the event so as to aid in proper investigation of your concern.

The Hospital encourages employees to openly discuss issues using the Hospitalís chain of command. However, if employees wish to make suggestions/comments anonymously, each employee should do so in a constructive and professional manner. Employees are protected against retaliation for reporting issues of concern. If you experience any retaliation for reporting, please inform the HR Director or the Compliance Officer.
